An experienced learning disabilities nurse aiming to support the well-being and social inclusion of people with learning disabilities by helping to improve their physical and mental health. Experience of working closely with psychologists, social workers, teachers and general practitioners to help clients to developing independence and community integration.
Focused on ensuring that the care, welfare and safety of all patients is of the highest possible standard. Jane is currently looking for a position that offers training as well as career progression opportunities over time.

LEARNING DISABILITIES NURSE          June 2008 - Present

Provision of care management services to Adults with learning disabilities. Managing a caseload including a wide range of service-users with varying and often complex issues.


  • Undertaking Risk Assessments.
  • Reviewing care packages and making adult placements.
  • Working within a multi-disciplinary team environment.
  • Providing specialist advice, support and training to other agencies.
  • Responsible for the management and clinical care of clients.
  • Providing appropriate therapeutic contact with clients, families.
  • Updating and maintaining appropriate nursing and joint records, care plans.
  • Liaising with clients G.P to update them on progress.
  • Organising home visits.
  • Advising clients on health education and preventative measures.
  • Responsible for making referrals to other professionals and agencies.


  • Using expert communication skills to engage with vulnerable people.
  • Experience of working in a residential setting providing 24 hour care.
  • Knowledge of attending health team briefings and joint team meetings.
  • Ability to organise and prioritise work.
  • Experience of completing management plans and reports.
  • Able to assist with tests, evaluations and observations.
  • Ability to work on  own initiative but also as a team player.
  • Involved in the education, support and performance monitoring of junior staff and students.
